Instructions

• One copy, signed, of the Agency/Client Terms for Collection.

• One copy of the Client Information Form.

• One Debtor Listing Form or one Judgment Listing Form for each case submitted.

• Any copies of bills, invoices, tenant applications, leases, judgments you may have. Include anything you feel may be pertinent to the case.

• If you are submitting bad checks, please submit originals.

• Please make sure you advise us of any action you have taken on the cases, judgments or checks you are submitting.
